Call Star Program @ (1-866-912-6283) if you are unable to get an appointment with a primary care provider within the 3 days.
Make sure to take the Form K-905-2403 [Medical/ Dental/ Vision/ Hearing examination] paperwork. As well as the medical consenter Form 2085 B (Provided by caseworker.)
